Updated Shrimp Declaration Form DS-2031

On April 17, 2024, the Department of State issued a revised DS-2031 Shrimp Exporter’s / Importer’s Declaration form. 

The new form is available on the Department of State website: https://eforms.state.gov/Forms/ds2031.PDF

The use of prior versions of the DS-2031 form should be discontinued, and the revised form should be utilized. The revised form is approved for use through April 30, 2027.

The form must accompany all shipments of shrimp and products from shrimp into the United States. It should include original signatures and be retained by the U.S. importer for three years from the date of import. It is to be produced upon a request from U.S. government officials.
